FM/Emergency Med. Match (R-3)
Application Process - Supporting Documents

Updated June 28, 2009
There are three major categories of supporting documents for an application file to a program participating in the Family Medicine / Emergency medicine R-3 match:
- personal letter
- letters of reference (with FORM)
- letter from your family medicine program / site director (with accompanying form)
Additional documents can be added such as a photograph, abstracts etc.. Most programs will specify in their program descriptions, any additional documents they require.
Please note that a custom resume is different than the Applicant Webstation (AWS) application; custom resumes can only act as supplements and do not replace the standardized CV/application.
Program descriptions are posted on the CaRMS website. Please go back and check updates after July 10 (see Timetable). Applicants should read the descriptions of programs of interest by going to the Program Directory on the CaRMS website. New programs may be added and modifications may be made during the year so keep reviewing the Program Directory. The "Supporting Documents" box on each program description lists the documents which are required by the program. Follow these descriptions carefully.
The personal letter :
is a letter to be written using the CaRMS form provided in AWS that outlines the reasons for you to choose an EM additional skills training program.
References:
In this Match, two types of references are requested and each type corresponds to a letter AND a specific form that you are able to print from our website (see below for the links).
- A letter from your program/site director accompanied by the relevant assessment form.
Letters of reference sent to CaRMS for processing, will require a coversheet. Each unique document requires its own coversheet. Coversheets are created in the Applicant Webstation (AWS) under the "My Supporting Documents" heading.
Reference letters should be sent directly to the CaRMS office, to the address on the "Letter of Reference Coversheet".
APPLICANTS REQUESTING A LETTER BEFORE AWS OPENS FOR THE EMERGENCY MEDICINE / FAMILY MEDICINE MATCH
You can start printing these forms and hand them out to your referees and Program Directors now to allow enough time for them to write their letters and fill the appropriate forms.
However, it is strongly advised to instruct your referees to wait AFTER August 26, 2009 before sending them to CaRMS.
When AWS opens, you will be able to create coversheets for these references, send them to our office and ease the work on our side to match the coversheets with the documents received. CaRMS will then be able to scan your documents in the appropriate spots you made available when creating your coversheets in AWS.
